provides home-based medical care to chronically ill patients, many
of whom are ill-equipped to navigate our overwhelming healthcare

provided by RN Nurse Care Managers, Social Workers, Pharmacists,

Coordinator -role is a critical member of the Landmark
-Interdisciplinary Team. This role reports to a Supervisor or the
Director, Health Services. Responsibilities are to ensure a
positive patient experience with -Landmark, as well as to build
good relationships with local provider groups to ensure that
-Landmark -is a reliable and trusted partner in patient care. To
perform this job successfully, an individual must be able to
perform each essential duty satisfactorily. The requirements listed

-Employees will be required have flexibility to visit the office at
least once per month for team meetings and/or training. This
position is full-time Monday - Friday. Employees are required to
have flexibility to work any of our 8-hour shift schedules during
our normal business hours of 8:00am - 5:00pm EST. It may be
necessary, given the business need, to work occasional overtime.
Employees will be required have flexibility to visit the office at

some days onsite and some days from home. *All Telecommuters will
