This role focuses on delivering exceptional nursing and administrative support within our personalized home health solutions, ensuring high-quality care and optimized patient outcomes. The ideal candidate will have a strong background in clinical health management, excellent patient interaction skills, and the ability to lead and train staff effectively. This position requires a proactive approach to patient care, coordinating with other healthcare professionals to ensure comprehensive service delivery. Develop and implement personalized care plans for patients. Manage and supervise a team of healthcare professionals. Ensure compliance with healthcare regulations and standards. Monitor patient progress and adjust care plans as necessary. Conduct regular staff meetings and training sessions. Collaborate with other healthcare providers to coordinate services. Maintain detailed patient records and prepare reports on care activities. Strong clinical skills and knowledge. Excellent leadership and team management skills. Effective communication and interpersonal skills. Ability to work under pressure and manage complex situations. Proficient in medical software and patient management systems. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN license. Certified Case Manager (CCM) or similar certification is preferred.
