Job Description:
HCA Florida North Florida Hospital seeks an experienced and
compassionate Adult Oncology Nurse Navigator to join our dedicated
healthcare team. This role is pivotal in providing specialized
nursing care and coordination services for adult oncology patients.
As part of our state-of-the-art healthcare facility, the Nurse
Navigator will work closely with patients, families, and
multidisciplinary team members to optimize patient outcomes through
individualized guidance and care plans. Assess patient needs and
work to create and implement personalized care plans. Serve as a
liaison between patients, family members, and various healthcare
professionals. Educate patients and families about treatment
options and health management strategies. Coordinate diagnostic
tests, treatments, and consultations to streamline care delivery.
Monitor patient progress and adjust care plans as needed. Advocate
for patients’ health and well-being within the healthcare system.
Facilitate patient access to community and healthcare resources.
Strong interpersonal and communication skills In-depth knowledge of
oncology care processes Ability to coordinate complex care plans
Effective patient education and advocacy capabilities Critical
thinking and problem-solving skills Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Registered Nurse
(RN) license in the state of Florida Oncology Certified Nurse (OCN)
