The Home Health, RN manages a caseload : In addition to hands-on care, the Case Manager oversees the activities of the multi-disciplinary team that provides care to the patient Monitoring the provision of therapeutic services through written and verbal communication with all disciplines Ensures that all visits scheduled and provided are authorized and covered by physician orders Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits Informs the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor Home Health Case Manager, RN specific skills: --- Interpersonal skills --- Educator skills --- Critical thinking and delegation skills --- Knowledge of regulatory standards appropriate to position --- Written and verbal communication skills --- Computer skills appropriate to position About BayCare:As one of the largest providers of home care services in the State of Florida, BayCare HomeCare has provided quality home care to Florida residents since 1976. Qualifications: Certifications and Licensures: --- Required BLS (Basic Life Support) --- Required RN (Registered Nurse) License --- Required Driver's License State of Florida Education: --- Required Associate's or Diploma Nursing --- Preferred Bachelor's Nursing Experience: --- Required 1 year Registered Nurse or 1 year Licensed Practical Nurse --- Preferred 1 year Home Care
