Job DescriptionFunction of the JobThis position is under the direct supervision of the Assistant Administrator of The Groves Assisted Living. Responsibilities include providing direct care to residents in both Cypress and/or Magnolia buildings. Essential Job ResponsibilitiesFollow established performance standards of nursing practices and implement facility policies and procedures.Interpret existing policies and procedures to nursing assistants, residents, families and physicians.Supervise and evaluate all direct resident care and initiate corrective measures as necessary.Obtain report from nurse that is being relieved and record sufficient information to implement appropriate follow-up action as necessary.Provide report to nurse coming on duty including sufficient information for follow-up action if necessary.Conduct resident rounds daily, reporting problems to the Nurse Manager and initiating corrective measures as appropriate.Identify safety hazards and initiate corrective measures.Identify resident problems and emergency situations and initiate immediate "life saving" measures in the absence of a physician, unless otherwise indicated.Follow, supervise and evaluate the implementation of the Resident Rights Program.Provide clinical supervision of nursing assistants providing direct resident care.Attend staff meetings, in-service classes and committee meeting as assigned.Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident population.Assume responsibility for compliance with federal, state and local regulations.Counsel nursing assistants and recommend corrective counseling, when necessary, to the Nurse Manager.Participate in the employee evaluation process by providing information on the clinical performance of nursing assistants to the Nurse Manager.Administer and document medications and treatments per the physician's order and accurately record all care provided.Perform and document comprehensive assessment of each resident.Observe, evaluate and document resident care provided, changes in resident's conditions and resident's response or lack of response to care provided. Report changes in resident's conditions to a Physician, Nurse Manager, and responsible party and take follow-up action as necessary.Notify physician, when necessary, of changes in residents' conditions, weight changes, medication errors, medication not available, abnormal lab and x-ray results and any other information as necessary.Follow facility admission procedures and communicate resident admission to other departments, physicians and ancillary service providers.Have knowledge and competency to perform basic nursing skills.Follow facility discharge/transfer procedures, coordinate discharge/transfer with other departments, physicians and ancillary providers and complete documentation.Assess resident needs and initiate development of individualized plans of resident care.Provide direct resident care and give instruction to nursing assistants related to resident care.Listen to resident, family or physician concerns and initiate corrective measures and/or report concerns to Nurse Manager.Supervise response to resident's calls for assistance.Count all narcotics, sign for count and exchange keys.Make report to nursing assistants and explain all assignments.Check meals to ensure proper diet is served and residents are assisted as necessary.Prepare and administer medications and treatment as ordered.Supervise application and removal of all postural supports.Order medications as needed, receive medication delivery from pharmacy, check all drugs received, and document proof of medications received, according to facility procedure.Administer PRN medications and treatments as necessary.Supervise passing of nourishment.Chart nursing notes for significant changes or when indicated.Review Pharmacy Consultant's recommendations and present to physicians for their decisions.Consistently work cooperatively with residents, charge nurses, other nursing assistants, staff, physicians, families, consultant personnel and ancillary service providers.Personally obtain continuing education relating to assisted living/long-term care fields of nursing.Observe infection control procedures.Responsible for the safety of residents under supervision.Follow Resident's Rights policies at all times.Observe all facility safety policies and procedures.Come to work as scheduled and consistently demonstrate dependability and punctuality. Come to work in a clean, neat uniform and consistently present a professional appearance. Report to the Nurse Manager, Director of Compliance or Compliance Hotline, any questionable conduct or suspected violation of regulations or Alliance Community for Retirement Living (ACRL) policy covering the delivery and/or billing of healthcare services.Assume accountability in the employees' handbook.Perform related and special duties as assigned by the Nurse Manager or Director of Health Care Services.QualificationsCurrent licensure/registration with the State Board of Examiners.Possess organizational, prioritizational and observational skills.Ability to implement interventions for patient care based on standards of nursing practice and principles of nursing care.Basic Life Support certification.
